Usual driver updates (ufs, lpfc, target, qla2xxx) plus assorted
 cleanups and fixes including the WQ_PERCPU series.  The biggest core
 change is the new allocation of pseudo-devices which allow the sending
 of internal commands to a given SCSI target.
